My CF Care Organizer 

Cystic fibrosis is a very personal disease and can affect a patient and impact a family to many different degrees. Successful treatment can often depend on the family and the patient’s ability to manage the complicated treatment process.

My CF Care Organizer was created, written and organized by family representatives involved with the CHEO CF Care Family Forum. This binder is designed to help CF families and patients in managing their care over the long term.

The CF Care Family Forum is an education and family support program initiated by your Cystic Fibrosis Canada Ottawa Chapter and developed in partnership with the CHEO CF Care team. Its purpose is to help families and persons with CF in taking charge of their health and care management. It is designed to empower families and persons with CF to become actively involved in their care, learn and acquire self-care skills and better manage all that is involved to help them remain healthy for as long as is possible.

This binder is a tool that will assist patients and their families in keeping track of their health records and will aid in effectively communicating and becoming “partners in care” with CF specialists and other health care providers.

It helps with keeping track of your daily treatment plan, symptoms, medications, setting goals, and preparing for your various doctor visits. It provides you with simple tracking sheets for knowing your medications and all your tests, and communicating with those around you of your daily schedule and needs.

This organization binder is meant to be a helpful tool for people and families living with cystic fibrosis - a condition where being organized and actively involved in your care can mean longer health and well being.
